Congratulations on choosing Dr. Steven Timm, DMD, as your dentist! We are excited to have you and your family as our new patients and will do everything possible to make sure each visit is comfortable.
At your first appointment with our office, we will evaluate your health history and perform a thorough clinical exam, including necessary X-Rays. We will discuss our findings and answer any questions you might have regarding your dental health. We will also recommend any treatment needed to ensure we keep your smile healthy.
To help your first appointment go more smoothly, we ask that you print our New Patient Information form and bring it with you on your scheduled visit. Simply click the link below, print the pages, fill them out and bring them with your insurance information (if applicable).
